Suffolk County Sheriff's Department Medical Alert Cards CANCELLED

The Suffok County Sherrif's Department will be at the Library to issue  "In Case of Emergency Cards" which can provide important information to medical personnel in the event you cannot speak for yourself.   You will need to complete a medical alert card worksheet and bring the completed form to your appointment.  The card will include information about your medical ailments, doctor's contact information, emergency contact and blood type.  An application is available at the Library or it can be emailed to you, but you will need to call or stop by the Reference Desk to make an appointment
